New horizons in combined surgical-orthodontic treatment of dentofacial deformity

Aim: To correct facial asymmetry with conventional orthognathic surgery versus distraction osteogenesis treatment protocol for the correction of facial deformity ensuring optimal form, function, esthetics and stability.

Methods:​ Case series including patients from post traumatic deformity, hemifacial microsomia, condylar hyperplasia to tmj ankylosis corrected with different protocols. Patients with hemifacial microsomia were managed with distraction osteogenesis protocol followed by orthodontic treatment. Team work provided us with the opportunity to achieve greater results ensuring greater stability to the surgical procedure. Tmj ankylosis was managed with Kaban and Shah protocol in children (a modification of kabaan). 

Results: The combined orthodontic – surgical treatment conferred good esthetic, functional and occlusal results. The asymmetry was corrected and the dentition finished in a class I relationship. No major post-operative complications were reported. 

Conclusions: ​Based on the achieved outcomes of our cases, distraction osteogenesis along with comprehensive orthodontic therapy displayed enormous potential for facial deformity correction. This combined treatment modality proved to be more efficient as well as ensured stability to the surgical result. Compared with conventional orthognathic surgerytreatment, distraction and orthodontic treatment presents greater potential for correction and stability specially in adult patients with hemifacial microsomia and tmj ankylosis.
