Accidental insertion of NG tube in trachea in intubated patient during surgery and ventilator malfunction

Nasogastric tube (NGT) insertion is a common practice in hospital wards, Intensive Care Units, and operating room for the delivery of feeding and/or medications to patients of all ages. The technique is simple in awake patients who cooperate by swallowing it, but occasionally, there may be demand to insert in unconscious or even in an intubated anesthetized patient. This procedure may lead to adverse events. We present a case of Accidental insertion of nasogastric tube NG into trachea leading to anaesthesia workstation malfunction which was inserted in an unconscious patient following endotracheal intubation.
